Is there a foam insert on the back and if so, is it up high or on lower back. If it is low, it would interfere with a small kayak seat.

Yes. There is an insert. I bought it to use in a kayak. It seems like it should work well but I have not had the opportunity to try it yet.

Does it float on the water?

If you order it with foam it does. When you place your order there is an option “with or without foam”. Order it with foam and it floats, otherwise it is just a fishing vest.

The sizing is somewhat vague, I would need a xxxl vest as I'm about 255 lbs and 6'2" is there anyone close to my size that could tell me if it adjusts

First I’m nowhere near your size. However, the vest is adjustable. I’m not sure it would adjust that much though.

Is there a spot for fish?

Not one I would use.
